  1. Saw your afrezza instagram. From the insert I see that it:

-- is inferior to lispro at lowering A1C in type 1 diabetes
-- is only non-inferior to lispro in type 2
-- has the SAME time of effect onset as lispro
-- has the SAME effect on blood glucose as lispro for the first hour after dosing (see PD graph in the insert)
-- has a risk of DKA that is three times that of lispro
-- impairs lung function for as yet no known reason
    -- requires regular spirometry
-- has a black box warning
-- has warnings of lung damage, COPD, cancer and two years latere and the FDA mandated long term safety studies have still not started

Things I have observed from reading various blogs, facebook, twitter, etc:

-- has basically NO insurance coverage
-- users report needing to use 3-4 TIMES as much as their old lispro dose with some reporting needing 15-20 inhalations a day
-- non-CGM users need up to three times as many -- or more -- ingersticks a day because afrezza is so rapidly metabolized, hyperglycemia from late digested carbs is a serious problem
-- had the worst launch in the history of pharmaceuticals last year and couldn't even outsell exubera
-- diabetics don't seem interested in it
-- many pharmacies have quit stocking it due to supply issues

About the only good thing I can find is that if someone does have true needlephobia, it might be an option worth considering, and it might be useful as a backup in case of emergencies.
